Change of Address Form

Moving? We are making it even more convenient to keep your credit union information up to date. You can also easily update your address online through Online Banking. Simply log in and click on Settings, which is located on the top right of the screen and click on Profile. Under Profile, click Address and enter your current mailing address.

Otherwise, you can use the Change of Address Form for address, phone number, and email address changes. You may turn in the form at your nearest branch location. Please note that this form must be notarized if it is not signed in the presence of a Maui FCU member service representative.

Parental Authorization for Minors

Use this form if the member or joint owner is someone other than the parent/legal guardian. Parental consent must be obtained. Please note that this form must be notarized if it is not signed in the presence of a Maui FCU member service representative.

Use this form if you are a joint owner on an account and wish to be removed from the account. Please note that this form must be notarized if it is not signed in the presence of a Maui FCU member service representative.

Social Security Direct Deposit Form

Your Social Security checks can be directly deposited into your Maui FCU account. Not only is it safe and easy, but you’ll have immediate access to your funds.

By Mail – complete Direct Deposit Form and send to Go Direct Processing Center, U.S. Department of Treasury, PO Box 650527, Dallas, TX 75265-0527
